Waiver Case Management

 

This service locates, coordinates, and monitors services and informal community supports provided to individuals.  The service ensures the health and safety of individuals consistent with their assessed needs.  Waiver Case Management provides assistance with establishing financial and functional eligibilty through individual service plans.
