THE MAPC PROGRAM IS A MEDICAL ASSISTANCE CARD FUNDED
PROGRAM.

Administered through The Human Service Center

MAPC workers overseen by a Human Service Center
nurse who makes regular home visits and provides ongoing monitoring

Allow persons with significant health issues to be
served in a less restrictive environment, such as Adult Family Homes
-
Services are medically oriented activities provided
to recipients to assist them with activities of daily living
-
Personal Care services must be provided in the
individual's home

Assistance with bathing
-
Assistance with dressing/undressing, toileting
and personal hhygiene

Accompanying recipient to medical appointments
Due to the limited capacity of this program, only
consumers receiving services through the Developmental Disabilities
Department of The Human Service Center are served. If you believe
your may be eligible for MAPC services and are not currently receiving
services through the department, we may be able to assist you with a
referral to another agency.
