WHAT IS MENTAL HEALTH OUTREACH?

The Mental Health Outreach Program can offer assistance in developing/maintaining independent socialization through individualized services for persona with serious mental illness.

 Available Services May Include:

Service Coordination
Assistance with daily living skills
Social/recreational activities - group and individual
Linkage to financial services
Education about mental illness for individual and family
Linkage to educational and/or employment opportunities

Who is Eligible?

Anyone is eligible who is 18 years and older and who experiences a major mental illness, such as schizophrenia, a bi-polar disorder or major depression, which has required repeated acute treatment and which has interfered with completing school, getting and keeping a job, living independently, and having satisfying personal relationships.
